The Praxis Knowledge Base is available to anyone irrespective of ownership of the product. Feel free to download any information and use it in your own practice.

By downloading this information, however, you enter into a contract with Infor*Med Medical Information Systems, Inc.

Please read this contract carefully and consider whether you wish to participate.

In essence, you promise:

1. Not to sell or license the material contained herein to third parties.

2. To become fully responsible for the material you download. Infor*Med or the authors do not warrant any accuracy as to the contents of the material.

Read the following terms and conditions carefully before going on to the next screen. This is a legal document between Infor*Med Medical Information Systems Inc. ("Infor*Med"), a California Corporation, and you ("the User)".
Infor*Med will license the knowledge base software contained herein to you only upon the condition that you accept all of the terms and conditions contained in this license agreement.
If you accept these terms and conditions, indicate so by selecting the " ACCEPT" option below.
If you do not, select the " DECLINE" option below.

License Agreement/Company Code of Privacy Infor*Med Medical Information Systems, Inc. is a major developer and seller of information systems. Praxis® Electronic Medical Records, and the FascoderÔ are Copyright trademarks of Infor*Med Corporation. (The number one selling EMR program for small and medium size clinics in the United States.)

One of the many features of the Praxis Electronic Medical Record System is to automatically export and import typical clinical information data, which is devoid of patient demographic information. This area of our Website is intended for doctors all over the world to download clinical information that may help their patients, regardless of whether they have or have not purchased a Praxis license.

By completing your registration you become a user of the clinical information contained therein, you (the "User") agree with Infor*Med Medical Information Systems, Inc., a California Corporation, to be bound by the terms and conditions as set forth in this Agreement. Read this Agreement carefully. You will be bound by its terms each time you access Infor*Med sites.

1. License

Infor*Med grants, and the User hereby agrees to a non-exclusive, non-transferable, revocable license, that permits the use of all data and documentation contained in the Infor*Med Websites in accordance to the terms and conditions of this Agreement.

License, subscriptions and online registration are granted to individual healthcare providers to be used on their own medical practice and/or as information for their patients.

Licenses and subscriptions for any other professions, organizations or individuals interested in this product can be arranged through Infor*Med Medical Information Systems, 818-592-2900.

2. Termination

The User may terminate this license at any time by notifying Infor*Med in writing. Infor*Med may at its sole discretion terminate this license at any time and for any reason, with or without prior notification, also in all cases where the User fails to comply with any of the terms and conditions of this agreement.
This could be done by deactivating the User's username and password and/or suspending systems operations. Infor*Med reserves the right to modify or terminate partially or totally this agreement, as well as services and/or product offerings at any time without notice to the User.
Infor*Med reserves the right to stop giving this service for free at any time.

3. Copyright and Restrictions

All data found in the Infor*Med a files, as well as all the documentation and software contained therein, is the property of Infor*Med Medical Information Systems, Inc. or its Licensors, and is protected by copyright and other intellectual property laws. Information received through Infor*Med is to be used solely for the individual purposes stated in this agreement. None of the content of Infor*Med Websites may be reproduced, transcribed, stored in a retrieval system, translated into any language (including computer language), retransmitted in any form or by any means (electronic, mechanical, Photostats, recorded, faxed, etc.), nor can it be resold, or redistributed without the prior written consent of Infor*Med.

It is however understood that the User may reproduce any part or all of this data for personal and clinical use only, and limited to one physical location where the User performs his or her regular work:

This copyright information has to be printed in a font size no less than 9 points ; and must be in each printed page given to the patient of all material that you download, irrespective of any changes you may make to same.

The user, is permitted, indeed encouraged, to make any changes as you see fit in the data, the original copyright notice must still be shown, as it is a derivative work.

4. Databases Maintained by Infor*Med

While great care has been taken in organizing and presenting the material placed in the Infor*Med Websites, the company does not warrant or guarantee any of the methods, descriptions or products described, nor any medical device information contained therein, as the company does not perform any independent analysis in connection with any of the product described , recommended or posted in the site. In addition, you agree to hold Infor*Med and any of the authors not liable for any advice, or approach described, suggested or implied herein.

Infor*Med does not assume, and expressly disclaims, any obligation to obtain and include any information other than that provided by its third party sources. It should also be understood that by making this material available Infor*Med is not advocating the use of any product, diagnostic or therapeutic approach or procedures as described therein, nor is it responsible for misuse of the information or procedure due to typographical errors.

5. Protection and Security

The User shall take all reasonable steps to ensure that no unauthorized person shall have access to Infor*Med.
The User shall not divulge, sub-license, assign, sell or transfer to any third party the user name and password established during registration. The User understands that provision of the user name and password will be required prior to accessing the information contained herein. The User may choose to allow Infor*Med's system to set a cookie containing username/password preferences on the User's computer for convenience. A cookie is a message given to a Web browser by a Web server. The browser stores the message in a text file called cookie.txt. The main purpose of cookies is to identify users and prepare customized Web pages for them. When you enter a Web site using cookies, you may be asked to fill out a form providing such information as your name and interests. This information is packaged into a cookie and sent to your Web browser, which stores it for later use. The next time you go to the same Web site, your browser will send the cookie to the Web server. The server can use this information to present you with customized Web pages.

Infor*Med Medical Knowledge pages are provided solely for educational and informational purposes, and are not meant to provide professional medical advice, counseling, or services.
Infor*Med does not, in real time, verify the credentials of all individuals representing themselves as medical professionals. The views expressed by our Infor*Med members do not necessarily reflect the views of Infor*Med Medical Information Systems. If you need medical services always contact a licensed professional in your area. Always follow the advice of your physician or other health care professional in regard to treatment, information and considerations.
You are entirely liable for all activities conducted through any names registered to your e-mail address ("Account").
The following Rules apply while using Infor*Med's Knowledge Bases:

A. You may not post messages to this knowledge base unless approved by Infor*Med staff. Such approval does not constitute any form of endorsement of your diagnosis, procedure or therapy. B. By submitting Content to any part of Infor*Med Website, you automatically grant or warrant that the owner of such Content has expressly granted Infor*Med a royalty-free, non-exclusive right and license to use, reproduce, modify, adapt, publish, translate, create derivative works from, distribute, perform, transmit and display such Content (in whole or part) and/or to incorporate it in other works in any form, media, or technology now known or later developed for the full term of any Rights that may exist in such Content. You do have the right to edit or remove the material from this web by providing Infor*Med with a 30 day notice to do so. In addition, Infor*Med has the right not to accept any editing you have done on the material for which you hold a copyright.

C. You, and any persons who have access to your Account, must evaluate and bear the risk associated with the accuracy, completeness, or usefulness of any Content.

D. In cases where you have allowed any other individual to use your Account, or have made your password publicly available, you recognize that you are fully responsible for and must indemnify Infor*Med for any liability arising from: (i) the online conduct of such user; (ii) controlling the users access to and use of Infor*Med material; and (iii) the consequences of any misuse. You further recognize that you, as the holder of the Account, are solely and entirely responsible for activities conducted through such Account.

8. Privacy Policy/Disclosure of Member Information to Third Parties

For visitors to both the public and private areas of our Web sites, our Web server may recognize the consumer's domain name, and the e-mail address (where possible).
We collect the domain name and e-mail address (where possible) of visitors to our Web page, the e-mail addresses of those who post messages to our bulletin board, the e-mail addresses of those who communicate with us via e-mail, aggregate information on what pages consumers access or visit, user-specific information on what pages consumers access or visit, and information volunteered by the consumer, such as survey information and/or site registrations.
The information we collect is used for internal review. Used to improve the content of our Web page or to personalize the content for the User. Used to notify consumers about updates to our Web site. Used by us to contact consumers for marketing purposes, and used to provide promotional information about new products and special offers. Infor*Med will not share users' credit card numbers, DEA numbers, or any other personally identifiable information.

Occasionally, Infor*Med sends e-mail messages to registered users to inform them of features and services available on Infor*Med. In the future, Infor*Med may contract with a bulk e-mail provider who may have the right to share the e-mail addresses of Infor*Med's registered users with third parties. Infor*Med acknowledges that some users may not want to receive such e-mail. To prevent us from sharing this information in the future, please indicate this preference upon registration with Infor*Med, or change the "Receive E-mail" setting in your user profile.

If you supply us with your postal address online you may receive periodic mailings from us with information on new products and services or upcoming events. If you do not wish to receive such mailings, please let us know by sending a request via e-mail to pdr.net@medec.com or by writing to Customer Service at 6320 Canoga Avenue, Suite 1500, #195, Woodland Hills, CA 91367. Please provide us with your exact name and address. We will be sure your name is removed from the list.

9. General

If any provision of this Agreement is determined to be invalid under any statute or rule of law, the same shall be deemed omitted and the remaining provisions shall continue in full force and effect.
This Agreement shall be deemed to be made in the State of California and shall in all respects be interpreted, construed, and governed by and in accordance with the laws of the State of California applicable to contracts executed and to be wholly performed therein.
This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements, oral or written, and all other communications relating to the subject matter hereof. No amendment or modification of any provision of this Agreement will be effective unless set forth in a document that purports to amend this Agreement and both parties accept that hereto.

This Agreement is personal to you and you may not assign your rights or obligations to anyone.
